the following crirteria should be applied: Large cells with abundant granular eosinophilic cytoplasm, nucleus to cytoplasm ratio of 1:2 or more, nuclei round, large and vesicular and may have a prominent cherry red nucleolus with sharply defined cell borders Immuno: GCFDP-15 and Androgen receptor positive DDX: Oncocytic carcinoma - GCDFP-15 negative Hystiocytoid carcinoma- weakly eosinophilic cytoplasm and amphophilic cytoplasm Secretory carcinoma- dPAS positive for mucin, young age

Microscopically, the lesions are composed of small ducts with two rows of epithelium scattered throughout a fibrous stroma. The tubular lumina may be filled with amorphous debris. Horn cysts are often present near the surface, while squamous metaplasia can be present throughout. Some of the ducts have characteristic comma-like tails and there are often solid strands of basophilic epithelial cells throughout the stroma which are independent of the ducts. These findings are similar to syringomas of the skin, which usually occur on the face and axilla, neck, vulva, and ventral torso. Syringomatous adenoma of the nipple differs from a syringoma of the skin in that there may be invasion of smooth muscle bundles and perineural invasion. Extension into the subcutaneous tissue and of parenchyma of the breast is common. There have been reports of similar skin lesion. This was first reported in 1982 by Goldstein et al and called microcystic adnexal carcinoma Other reports have designated it as sclerosing sweat duct (syringomatous) carcinoma, malignant syringoma, desmoplastic trichoepithelioma, and trichofolliculoma. This tumor shows both pilar and eccrine differentiation. In general, syringomatous tumor of the nipple lacks cytologic atypia, has minimal mitotic activity, and only rare foci of necrosis. It tends to recur unless completely excised. Recurrence has been reported up to 30 years later. In a few cases, the recurrences are slow growing, but distant metastasis has not been reported. Complete excision, not mastectomy, with long term follow-up is the treatment of choice.

Note the marked pleomorphism of the cells with variation in size, shape and polarity. Signet ring cells are obvious as well as atypical mitosis and apoptosis.
