Mycosis fungoides (MF) is an epidermotropic, primary cutaneous T-cell lymphoma (CTCL) characterized by infiltrates of small to medium-sized T lymphocytes with cerebriform nuclei. Stages are Patch, Plaque and Tumor stage. Screen for Mycosis fungoides or Sézary syndrome in peripheral blood using flow cytometry for CD26 negative or dim expression

Interface dermatitis that shows: apoptotic individual keratinocytes, hydropic degeneration of basal keratinocytes, intercellular oedema (spongiosis), blisters within and under the epidermis/epithelium, epithelial/epidermal necrosis – but there are no large sheets of epidermal necrosis as seen in Stevens-Johnson syndrome / toxic epidermal necrolysis (SJS/TEN). This specimen is always RUSH and dont forget to make a phone call to the dermatologist. Many will do a frozen section.

Sites: Head and neck region and rarely in the distal extremities. Well circumscribed tumor located in the dermis and subcutaneous tissue ; No epidermal connection with the overlying epidermis.Tumour consists of epithelial component set in a chondroid, myxoid and fibrous stroma ; There are epithelial nests, islands, ducts and tubular structures . Epithelial cells are CEA and cytokeratin positive; The outer layer of ductal cells (myoepithelial cells) are Vimentin and S100 positive.

Acanthotic epidermis containing occasional necrotic keratinocytes. A patchy, bandlike infiltrate of lymphocytes, plasma cells, and histiocytes is seen in the papillary dermis. In addition, there is a superficial, deep, and perivascular dermal infiltrate.
